I missed seeing any WoW titling on it at all. It seemed to be "DC*MMO Survey" everywhere I looked. I've since gone back and found it in the drop box, though. Oops.

(And just in passing, a "-Select a Survey-" or similar as the default option would probably increase usability a little there - with an empty-looking drop box it's less intuitively obvious what a user should be doing.)

Unfortunately, I had to hack the PHP code to make this look a little better. Since there is (currently) only one survey, it made sense to make that the default option. (and remove a blank option... no need for that)
