Milestones - is there any use to them from a mechanics standpoint, beyond an action point reset if you complete two encounters in a single wakeful period? Is there a master list of them defined by the game itself, or is it done by the GM?

I'm afraid one of your commentators has either got it wrong or is citing a source other than the Player's Handbook / DM's Guide (I only have the core 3 books). Milestones are automatic. They occur whenever the team completes 2 encounters in a row without stopping for an extended rest, and all they do is give you one fresh action point (NOT a reset).

The DM can however deem some encounters more or less significant for milestone counting purposes, e.g. an easy encounter doesn't count, a hard encounter counts for 2.

Very useful, for Milestones = Action Points, and action points are made of a rogue doing VERY large amounts of damage.

Or, previously, my cleric taking the party from mostly hurt to almost completely healed, in just one turn.

I will say that "4th Ed" is a fun game, but it's not Dungeons and Dragons. This from someone who played BEFORE the first hardback books came out.
